What information do I need for the cholesterol intake form?

The cholesterol intake form asks about your health history, your current medications, your cholesterol information and your treatment preference, so the clinician can decide whether cholesterol management treatment may be appropriate for you.

What to have ready

  • Your ID

  • A list of your current medications

  • Your current prescription, if you’re already taking a cholesterol medication

  • Recent cholesterol lab results from your own doctor, if you have them

About labs

Labs are part of cholesterol management treatment, at the clinician’s discretion: the clinician may ask for recent results you already have, or order labs for you, and will tell you what’s needed.

How the form fits in

You complete the Risk Assessment first, then one intake form at a time: if you also want weight loss treatment, that form comes after this one. When you submit the form, you sign up and pay for the GenMeds Membership, and the clinician typically reviews it within 1–2 business days.

Put your health details in the form itself, not in the help chat or an email: the Support team doesn’t handle health information, and the clinician works from what’s in your form. If you’re not sure whether something matters, include it.

For questions about how the form works, message the Support team from your Dashboard.
